Process Steps (Allow 21 days for
appointment processing):
No new agent appointments within the following counties at this time: Brevard, Broward, Charlotte, Citrus, Collier, Dade, Escambia, Hillsborough, Indian River, Lee, Manatee, Okaloosa, Palm Beach, Pinellas, Polk, Santa Rosa, Sarasota, St. Lucie, and Volusia.

Fill out the appropriate appointment forms linked below and forward to FMS
along with a check payable to BUI
in the amount of $124.20 for appointment fees.

FMS checks for proper completion and forwards to Bankers.

BUI enters the sub-agent information in their system and forwards a
username/password to the FAIA member.

FAIA member quotes, binds, and issues policies via Bankers' website

Commissions are paid directly to the sub-agent by EFT (electronic
funds transfer). This is the only option available via
this program.
Per Bankers, the application for
appointments is currently being updated and
is not available at this time. FAIA
will post the new application packet asap
once it has been
completed.
