Process Steps:

Fill out the appropriate appointment forms linked below and forward to FMS
along with a check payable to BUI
in the amount of $124.20 for appointment fees.

FMS checks for proper completion and forwards to Bankers.

BUI enters the sub-agent information in their system and forwards a
username/password to the FAIA member.

FAIA member quotes, binds, and issues policies via Bankers' website

Commissions are paid directly to the sub-agent by EFT (electronic
funds transfer). This is the only option available via
this program.
Coastal agent
appointment forms (applies if there is coastal exposure in the county
where the agency is located)
Inland agent
appointment forms (applies of there is no coastal exposure in the
county where the agent is located)
